“Institutionalized” starts the moment you enter The Institute elevator. You have to make your way through empty corridors while listening to the voice and eventually enter a room. In that room, you should meet Shaun and Father. Following the talk with the Father, you could choose between three options: join The Institute, postpone the decision and reject
- If you decide to join The Institute, go talk to Clayton Holdren, Allie Filmore, Justin Ayo, and Madison Li. After having the Pip-Boy modified, you can fast travel in and out of The Institute at will. You could then trigger the Synth Retention quest.
- Postponing the decision allows you to explore The Institute which grants you the best chance to complete either the Outside the Wire or Inside Job mission. All you need to do is to locate the terminal in The Institute relay area then load the network scanner holotape. After that, either Form Ranks or Liberty Reprimed will start.
- In the case that reject, you could leave The Institute normally. That being said, it’s impossible to explore The Institute as the areas beyond the Father are locked. Soon, Form Ranks is going to begin.
